Add Forms for your company

  • Login into your account

  • In the left menu click on Business Settings icon

  • In the top menu choose Class Configuration Settings Icon

  • Choose Forms icon

  • Click on New Form button

  • Under Form Title, write in appropriate title

  • Under Form Description, write in appropriate description

  • To create the form press Question button

  • In Question box write in your question

  • Under Answer type, choose the answer type you want to use.

  1. Text-gives parent an ability to give a short answer such as “yes”, “no”, “maybe”...

  2. Long text-gives a parent ability to give a more detailed answer

  3. Drop down-create answer sections for drop downs with +Add Option button, then press return to move on to next +Add Option.  Parent will be able to choose one of the options.

  4. Radios-create answer sections for Radios with +Add Option button, then press return to move on to next answer +Add Option.  Parent will be able to choose one of the options by checking off a circle next to the answer.

  5. Checkboxes-create answer sections for checkboxes with +Add Option button, then press return to move on to next +Add Option.  Parent will be able to choose several options as the answers.

  6. File upload-parent will be able upload any forms required (ex. Medical forms..)

  • Select Required if you require the from to be filled out

  • You have an option to duplicate the same question that was just written or press Add Question to add another question to the form

  • Press Save button to save the Services and goods

  • Press Cancel button to get back without making any changes
